FLEXIBLE SPENDING ACCOUNT

– Employees can set aside pre-tax money for medical and dependent care expenses up to a maximum of $5,000/year for each plan.

DISABILITY BENEFITS

– Short-Term Disability: Pays 60% of weekly earnings to a maximum of $1000/week; Benefit period of up to11 weeks; 14-day waiting period.
– Long-Term Disability: Pays 60% of basic earnings to a maximum of $6000/month; Benefits payable to age 65.

RETIREMENT PLAN

– Eligible employees may contribute to 401(k) after (1) one month of employment.
– Employees can contribute up to 100% of salary to a maximum of $17,500/year.
– CENTERS contributes 8% on an annual basis regardless of employee contributions.
